This is to remind all School Heads and Adopt-a-School Program (ASP) Coordinators regarding the online submission of the ASP School Partnership Data Sheet (SPDS) First Quarterly Report, which covers all forms of ASP donations and support received from January 1 to March 25, 2018.
A printed copy of the SPDS signed by the School Head and the District Supervisor, together with the required attachments (i.e. Memorandum of Understanding/Agreement, Deed of Donation, and Deed of Acceptance), must be submitted to the Division Office SGOD on or before March 28 (Wednesday). All schools are expected to comply with this deadline.
