1. At the end of every School Year, preparation and checking of school forms are among the critical activities conducted at the school level. With the issuance of DO 4, s. 2014 entitled “Adoption of Modified School Forms for Public Elementary and Secondary Schools Effective end of SY 13-14″ and DO No. 3, s. 2018 and DO No. 58, s. 2017 re: ” Adoption of New Forms for Kindergarten, Senior High School, Alternative Learning System, Health and Nutrition, and Permanent Records, streamlined and electronic school forms were introduced simplifying the process of preparation and checking of school forms!
