UM | SUBMISSION OF ADOPT-A-SCHOOL SPDS SECOND QUARTERLY REPORT

This is to remind all School Heads and Adopt-a-School Program (ASP) Coordinators regarding the online submission of the ASP School Partnership Data Sheet (SPDS) Second Quarterly Report, which covers all forms of ASP donations and support received from April 1 to June 30, 2018, not included in the Brigada Eswkela report on generated resources.
A printed copy of the SPDS signed by the School Head and the District Supervisor, together with the required attachments (i.e. Memorandum of Understanding/Agreement, Deed of Donation, and Deed of Acceptance), must be submitted to the Division Office SGOD on or before June 29 (Friday). All schools are expected to comply with the deadline.
