1. In reference to DM/OM-CI-2022-00272, titled “Onsite Administration of the Philippine Educational Placement Test (PEPT) and Other Regular Assessment Services”, this Office announces that the onsite administration to learners needing validation of grade level/s that were completed from schools without a permit pre-registration is now open.
2. All interested applicants may submit the following requirements at the CID Office (Monday to Friday), 1:00pm to 4:00pm only until October 31, 2022.
a. Birth certificate (Original and photocopy)
b. Permanent school records (SF 10/Form 137)
c. 2 pieces 1×1 pictures with name tag
3. The schedule of onsite administration of PEPT will be announced in a separate memorandum.
4. Attached is the Memorandum DM/OM-CI-2022-00272 for your reference.
5. Immediate dissemination of this Memorandum is earnestly desired.
