1. In reference with Regional Memorandum No. 20-483 dated October 21, 2020 re: Submission of LDM 2 Forms Number 4 for Regional Consolidation of Evaluation Results, the field is hereby directed to submit the LDM2 pre-implementation (module-based) outputs/ portfolios of teachers on or before November 6, 2020 to their respective LAC leaders. All teacher’s outputs/ portfolios shall be compiled by the respective schools, through the LAC leaders and coaches, for possible onsite validation.
2. In ensuring the successful completion and compliance of the participants to LDM Course 2, the LAC leaders and instructional coaches shall download and accomplished the prescribed consolidated reports template through the links below. Forms shall strictly follow the filename convention stipulated in the LDM 2 evaluation guidelines.
