1. In maintaining an updated personnel information, the field is hereby directed to accomplish and submit the updated Civil Service Commission Form 212 revised 2017, (Personal Data Sheet) for all government officials and employees.

2. To be included in the PDS are all learning and development interventions/training programs attended until February 28, 2019. The computerized CSC Form 212 must be subscribed and sworn by the following personnel:
a. School Principal/ OIC/ TIC – school-based personnel;
b. Mrs. Gladys G. Reyes, Administrative Officer V – School Division Office personnel and School Principal/OIC/TIC

4. The updated PDS must be submitted in two (2) printed copies with transmittal form on or before March 20, 2019 to the Personnel Unit c/o Mrs. Maricel A. Mendoza, Administrative Officer IV. It is understood that the school copy is filed in the personnel’s respective 201 file.
