This is to inform the field that the City Schools Division of Dasmariñas – Health and Nutrition Unit will be releasing a Medical Liability Waiver to be filled up and signed by the participant’s to any event that requires a medical certificate. This is to ensure that there will be full disclosure of the medical history and corresponding medications of the participant’s.
