In reference to DepEd Memorandum dated April 23, 2018 re: Baseline Data Survey by the Nielsen Company, ALL public elementary and secondary schools are enjoined to accomplish the attached templates immediately in preparation for the school visit and interview. School heads SHALL read the attached Guidelines in the Conduct of the Baseline Data Gathering for NSBI, DCP and DICP in Public for reference.
