1. In reference to Division Memorandum No. 542, s. 2024 titled “Dissemination of Office Memorandum Order No. 397-S-2024 (Pambansang Pabahay Para sa Pilipino (4PH) Program), this Office informs the field the updating of the 4PH Pre-Qualification Report of the Urban Poor Affairs Unit of the City of Dasmariñas.
2. The 4PH Pre-Qualification Report shall ONLY be accomplished by the concerned Administrative Officer/Administrative Assistant/Personnel-In-Charge of each school. The A0s/AdAs/ Personnel-in-Charge shall classify the names of those listed in the Pre-Qualification Report per school, input the necessary details per employee, correct errors if needed, and put the name of the assigned personnel who will make the necessary changes. The list of names shall not be rearranged. It is likewise requested for all the necessary details to be accurate and precise. The working file (link) for the Pre-Qualification Report and other details on the updating of the mentioned report will be disseminated in the group chat on Messenger.
