1. Pursuant to DepEd Order 016, s. 2025 titled Guidelines on the Grant of Medical Allowance to the Department of Education Personnel, this Office informs all permanent teaching, non-teaching, and related-teaching employees of the City Schools Division of Dasmarinas to fill out the readily available Google Form relative to the grant of medical allowance to DepEd personnel. The Google Form will be sent to DepEd emails of all concerned employees.
2. Moreover, permanent personnel of each school shall fill out and submit the hard copy of the Medical Allowance Registration Form (Annex A). Administrative Officers/Personnel-in-Charge shall collect and consolidate all the accomplished hard copies of Annex A. The deadline for online and hard copy submissions is July 23, 2025 (Wednesday).
3. The Administrative Office of the City Schools Division Office of Dasmarinas will serve as the Focal Office and be responsible for overseeing the implementation and providing technical assistance, guidance, support, and monitoring to ensure the successful implementation of DepEd Order 016, s. 2025. Furthermore, the Administrative Office shall determine the eligibility of all the permanent personnel on the grant of medical allowance, following the eligibility requirements as stipulated in the mentioned DepEd Order.
