1. In reference to Division Memorandum No. 559, s. 2025 and Division Memorandum No. 548, s. 2025 regarding the National Assessment for School Heads (NASH) FY 2025, this Office hereby announces the extension of the deadline for submission of documents by eligible personnel until August 20, 2025 at 2:00 PM, to be submitted at the Records Unit of the City Schools
Division of Dasmariñas.
2. Additional documents required for verification shall be submitted including the duly accomplished Personal Data Sheet (PDS) and the original copy of the Individual Performance Commitment and Review Form (IPCRF).
3. NASH test takers identified in the profile released by the Central Office who will not proceed and participate in the scheduled examination are required to submit a waiver indicating their non-participation. A personal letter addressed to the Schools Division Superintendent (SDS), indicating their reason for non participation, shall serve as the official waiver. If a template will be released by the Central or Regional Office, it will be forwarded promptly to the concerned personnel for their guidance and compliance.
