1.  In reference to DepEd Order No. 12, s. 2025, “Multi- Year Implementing Guidelines on the Calendar and Activities for SY 2025-2026”, this Office announces the administration of Multi-Factored Assessment Tool to Grade I learners, which assesses developmental status or any manifestations of learning difficulty.

2. The policy aims to identify learners with difficulties, who may need educational services and assist teachers in planning and designing instructions appropriate for learners with difficulties (LWDs).

3. This MFAT is a classroom activity- based assessment covering the five domains of learning:
a. Cognitive
b. Communication
c. Socio – Emotional
d. Psycho- Motor
e. Daily Living Skills

4. The deadline of submission of the MFAT report is on or before September 30, 2025, through SDO- Dasmarinas City Records Unit using the attached template.
