1. This is to inform all public and private schools in the Division that the submission of Learner Information System (LIS) Request Forms for data correction through Central Office Ticketing System will be transacted via email at planning.dasma@deped.gov.ph. Schools are reminded to submit the completed request forms in both excel and pdf formats, along with all required supporting documents as indicated in the respective forms.
2. The only official revised Request Forms (RFs) for data correction which can be downloaded from the LIS Support tab are as follows:

b) RFO1_LRN Merging
c) RF02_LRN Reactivation
d) RF03_Enrollment Data Issues
e) RF04_Unmerging of LRN
f) RF05_UAMS & School Concerns
g) RF06_Merging of School ID
h) RF07_Reopening of Enrollment & EOSY Finalization
i) RF08_Transfer Related Issues
j) RF09_Unenrollrnent of Learner
k) RF10_Updating School Calendar
