This is to inform all public and private schools in the Division that the submission of Learner Information System (LIS) Request Forms for data correction through Central Office Ticketing System will be transacted via email at planning.dasma@deped.gov.ph. Schools are reminded to submit the completed request forms in both excel and pdf formats, along with all required supporting documents as indicated in the respective forms.
2. The only official revised Request Forms (RFs) for data correction which can be downloaded from the LIS Support tab are as follows:
