1. Pursuant to Regional Memorandum No. 776, s. 2025, issued by DepEd CALABARZON through the Education Support Services Division, all public and private schools are requested to actively participate in the collection and consolidation of data on reported Learner Rights and Protection (LRP) concerns for School Year 2024-2025. This initiative is anchored on DepEd Memorandum OUOPS-2025-11-057701.

2. All schools are instructed to accomplish the School-Based Consolidation Data Sheet downloadable via: https://tinyurl.com/4vhn4sxf and submit the data through the designated Google Forms per school level:

3. Submission deadline is on or before December 15, 2025. All reports must include the accomplished data sheet and supporting documentation of reported incidents.

4. School Heads and Guidance Counselors/Designates are reminded to ensure accuracy, completeness, and confidentiality of submitted data, in accordance with DO 40, s. 2012; DO 55, s. 2013; and DO 18, s. 2015.
