1. Relative to Division Memorandum No. 440, s. 2025, titled “Disseminating School Health Activities for the School Year 2025-2026, all schools are hereby directed to conduct the Endline Nutritional Status Assessment of all K-12 learners and School-Based Feeding Program (SBFP) Beneficiaries. The assessment shall include the measurement of height and weight using the following indicators:
✓ Body Mass Index (BMI)
✓ Height-for-Age (HFA)
Reports submitted must be 100% complete.
2. Schools are instructed to use the BMI Software or School Form 8 (SF8) provided by the Central Office for the recording and consolidation of all assessment results.
