1. The correction of entries in the school records of a learner/student in the public and private schools within Dasmarinas is a process lodged with the Legal Unit of the Division Office. This includes correction in the name and date of birth of a student in the school record, as indicated on their Certificate of Live Birth issued by the Philippine Statistics Authority.
2. This Memorandum serves as notice to all schools in Dasmarinas, whether public or private, about the existence of this service, as well as to inform them of the process so that all personnel in the field will be guided accordingly.
3. Attached to this Memorandum are the requirements and the guidelines on the aforementioned procedure.
