1. The Office of the Undersecretary for Human Resource and Organizational Development and Infrastructure (OUHRODI) aims to consolidate grassroots-level information on school’s training needs as part of its organizational diagnosis. To attain to this objective of implementing an effective School-Based Management (SBM) system, school heads shall accomplish the SBM Leadership Training Needs Survey.
2. The survey form can be accessed at tinyurl.com/SBMFY26-SurveyForm until February 9, 2026.
3. School heads, as survey respondents, must ensure the accuracy and completeness of the information to be provided in the link by carefully reviewing answers prior to submission. Only one (1) submission per school head must be submitted. Only DepEd email address shall be used and declared for this purpose.
