1. Relative to a letter received from Mr. Jose Luis F. Decena, Social Welfare Assistant of City Operations Office (C00) – Dasmarinas, this Division informs all public elementary and secondary schools that the Field staff of 4Ps in Dasmarnas City will distribute the Compliance Verification Forms (CVFs) in a soft copy format through a google link to be sent on the official 4Ps Coordinators group chat.
2. CVF is a tool used in monitoring the educational condition of the program in public elementary and secondary schools. Thus, all 4Ps school Focal persons are expected to facilitate the accomplishment of the said CVFs and shall submit the same to the SGOD Office on or before July 09, 2026 (12NN).
