As stated in the Division Memorandum dated June 23, 2016, re: Official Format of Form 137 for Elementary Schools, paragraph 1. that explains:
“Attached herewith is the official format of the Form 137 (ELEMENTARY SCHOOL PERMANENT RECORD). In this regard, all Public Elementary School Heads are advised to begin implementing the use of said Form 137 starting School Year 2016-2017.”
It is hereby corrected as:
The Official Format of Form 137 (Elementary) specifically for Kindergarten is the Format based on the Kindergarten Progress Report Card as reflected in DepEd Order No. 47, s. 2016 re: Omnibus Policy on Kindergarten Education.
Attached herewith is the Format for Kindergarten Form 137.
Furthermore, the ECCD Card that are distributed earlier will be used as recording for ECCD assessment.
