1. Pursuant to DepEd Memorandum No. 114, s. 2017, Reiteration of the Reportorial Obligation of Schools, Schools Division Offices, and Regional Offices
pursuant to DepEd Order Nos. 40, s. 2012, 55, s. 2013, and 18, s. 2015, all public and private schools are directed to submit the following reports:
a. School-Based Report on Incidents of Bullying (SY 2022-2023);
b. School-Based Report on Incidents of Child Abuse (SY 2022-2023);
c. School-Based Report on Cases of Children-At-Risk (CAR) (SY 2022-2023);
d. School-Based Report on Cases of Children in Conflict with the Law (CICL)
(SY 2022-2023);
e. School-Based Report on Other Learner Rights and Protection Concerns; f. School-Based Child Protection Policy and Anti-Bullying Policy, certifying
localization of DO 40, s. 2012; and
g. List of Child Protection Committee (CPC) Members for SY 2023-2024.
2. All private and public schools shall submit the aforementioned report on or
before August 11, 2023, 5:00 PM, and shall upload it using this link: https://tinyurl.com/ConsolidatedLRPCases2022to2023. All forms shall be uploaded in PDF Format.
