1. Pursuant to the Memorandum issued by the Office of the Secretary, signed by Atty. Nepomuceno A. Malaluan, Chief of Staff, dated January 03, 2021 re: DepEd Citizen/Client Satisfactions Survey (CCSS) Results of Fiscal Year 2021 for the Grant of Performance-Based Bonus (PBB) for FY 2021 and a Requirement of the Anti-Red Tape Authority (ARTA) Relative to Citizen’s Charter, all concerned units in this Division Office and public schools are hereby requested to answer the Google Form which links are provided therein.
2. As additional directive in the abovementioned Memorandum, all schools are instructed to prepare the needed data and information regarding Citizen/Client Satisfaction Survey which are as follows:
