1. The Office of Undersecretary for Administration (OUA) through the Bureau of Learner Support Services – School Health Division (BLSS-SHD) and the Information and Communications Technology Service (ICTS), Information and Communication Technology Service (ICTS) issues the Guidelines on the Updating of Vaccination Status of Department of Education Personnel Through the Self-Reporting Covid Monitoring Application. This is relative to Division Memorandum No. 127 s. 2021 titled Mass Dissemination of Information on Self Reporting for Covid-19 Vaccination Tracker issued last July 21, 2021.
2. Based on the audit of data submitted through the DepEd Monitoring Application, there are some groups of personnel who submitted their vaccine status using only one DepEd email account. In this regard, the Health and Nutrition Unit nurses will contact those different individuals whose names appear under one DepEd account and will be instructed to re-submit their vaccination status.
3. In addition, all DepEd personnel of this division who still have not submitted their vaccination status must already do so.
4. Enclosed is DepEd OUA Memo 00-0122-0029 for your reference in submitting and re-submitting data.
5. Immediate dissemination of and compliance with this Memorandum is directed.
