1. In reference to DepEd Order No. 11, s. 2018, RE: Guidelines on the Preparation and Checking of School Forms, this Office informs all public schools of the composition of the Division Checking Committee (DCC) and the DCC Subcommittee (see Enclosure 1).
2. The Public Schools District Supervisors (PSDS) will serve that as the / Chairperson of checking the school forms in their respective clusters to ensure proper planning, scheduling, and adjustment if necessary, and to achieve timely submission of reports.
3. Select division and school personnel are deputized to be part of the DCC Subcommittee for a more efficient checking of public-school forms. This activity shall be conducted from March 16 to 27, 2026, subject to the final schedule to be determined by the Cluster Chair.
