1. In reference to Division Memorandum No. 132, s. 2026, titled “Division Checking Committee (DCC) for SY 2025-2026 and Schedule of Checking of School Forms”, this Addendum Memorandum is hereby issued to provide additional members of DCC Sub-committee.
2. The following school personnel are hereby deputized as additional members of Cluster 3 to address scheduling conflicts affecting some of the original members of the cluster:
a. Adela P. Sacay – Pag-asa National High School
b. Araceli A. Ramos – Pag-asa National High School
3. All other details on the said memorandum are still in effect.
