1. In line with the instructions of the undersigned to establish a system and control mechanism to track and to facilitate school submitted request letter for indorsement to the City Government of Dasmariñas, all public and private schools are hereby informed of the online form where all submitted and received letters shall be encoded.

2. The submitted request letter inked with official receiving stamp of the Division
Office, which is subject to the approval of the City Government of Dasmariñas
considering the alignment to approved LSB Plan of the year applicable, shall be encoded through the link below within 24 hours from the time of submission to the Records Unit:
