1. In compliance with DepEd Order No. 44, S. 2004, the City Schools Division of
Dasmariñas through Health and Nutrition Unit will be conducting the Annual Physical Examination of Teaching and Non-Teaching Personnel from July 2023 to August 2023.
2. All Teaching and Non-Teaching Personnel in the division must provide Chest X-ray and Urinalysis result during their APE scheduled date. All are encouraged to present additional Laboratory Results such as Fasting Blood Sugar, Lipid Profile, Cholesterol and Uric Acid results especially personnel with Pre-Existing Health Condition (ex. High Blood Pressure, Diabetes, Hyperlipidemia, Hyperuricemia, Cancer etc.) for better assessment and referral. The Health and Nutrition Unit shall not provide nor endorse any laboratory clinic.
3. Attached herewith is the schedule along with the format of the Annual Physical
Examination (APE) Masterlist. Encoded APE Masterlist must be submitted in soft copy thru our official email at hnu@depeddasma.edu.ph on or before June 30, 2023 with the title: “APE2023_School Name”. Please ensure that all teaching and non-teaching personnel are encoded in the master list.
