1. In line with the assessment of the Nutritional Status of all learners (K-12), the
field is hereby directed to submit their report on the baseline nutritional assessment of learners, specifically the height and weight.
2. In this regard, all learners shall undergo Nutritional Status Assessment,
particularly utilizing the Body Mass Index (BMI) and Height For Age (HFA).
Hence, all reports must be 100 % completed.
3. Schools shall utilize the Body Mass Index Software or the School Form 8 (SF8)
provided by the Central Office.
4. All reports must be submitted to Health and Nutrition Unit at hnu@depeddasma.edu.ph on or before September 27, 2023.
5. Enclosed is the template of the report for your reference.
6. Widest dissemination of and compliance with this Memorandum is directed.
