1. In reference to Regional Memorandum No. 140 s. 2025 titled “Advisory on the School-Based Deworming Program”, all schools are reminded to submit their accomplishment reports for the School-Based Deworming Program to the City Schools Division Office through the Health and Nutrition Unit.
2. For School Year 2025- 2026, Round 1 of the program is scheduled in July to September 2025. To ensure timely consolidation of reports and submission to the Regional Office, all school heads are directed to submit their accomplishment reports to the Health and Nutrition Unit on or before September 5, 2025. The schedule of activities, accomplishment targets, and submission deadlines are as follows:

3. Submit the accomplishment report to hnu@depeddasma.edu.ph with the subject line: “SB Deworming Report_Round [No.]_SY 2025-2026_[School Name]”
4. The Health and Nutrition Unit will strictly observe the set deadlines to ensure timely transmission to the Regional Office.
