1. Relevant to Division Memorandum No. 209 s. 2025 titled “Annual Physical and Dental Examination of Teaching and Non-Teaching Personnel”, this Division through the School Health and Nutrition Unit (SHNU), all School Heads of all Public Elementary and Secondary Schools are advised to make necessary call and decide to address the following teaching and non-teaching personnel who failed to comply and submit their APE requirements.
2. The following teaching and non-teaching personnel (See Enclosure No. 1) are given enough time to comply with all these requirements. However, despite continuous follow-up and coordination of SHNU to school clinic designate, they failed to complete and submit the minimum APE requirements, given more than 2 months to submit and comply. This is being done to ensure that all teaching and non-teaching personnel are non-infectious, and the school can safely operate.
3. All teaching and non-teaching personnel with pending APE requirements stated in Enclosure No. 1 must submit a letter of explanation addressed to the Office of the Schools Division Superintendent through their respective school heads.
4. All teaching and non-teaching personnel who are already reported to SHNU as on Maternity Leave (ML) or on Sick Leave (SL) due to chronic health condition (See Enclosure No. 2) shall submit their APE requirements upon or prior to reinstatement to school.
