This office directs all schools to implement the updated forms and checklists for Personnel transactions effective immediately. The forms and checklist shall be attached in the documents to be submitted to the Division Office to ensure the correctness and completeness of the documents.

The updated forms and checklists will be emailed to the schools’ official email address.

Immediate dissemination of and strict compliance with this Memorandum is directed.
