Appurtenant to DepEd Order No. 34, s. 2022 on “School Calendar and Activities for the SY 2022-2023” and in consideration with the provisions of the Learning Recovery Plan, this Office through the members of the Division Field Technical Assistance Teams (DFTATs) will spearhead the intensive monitoring of the full implementation of face-to-face classes on November 2 to November 4, 2022.
Attached herewith is the composition of the DFTATs (DM No. 493, s. 2022) with corresponding Clusters to be monitored and the Monitoring & Evaluation (M & E) Tool which shall be utilized for the said activity.
Furthermore, each school shall ensure the readiness and should prepare the following:
- Documents on school data such as enrolment (SF1), summary of classes organized, physical facilities (instructional rooms, laboratories), etc.
- Curriculum implementation documents such as class program, teachers’ loading (SF7), learning resources inventory and utilization report (print and non-print), etc.
- Health and safety documents such as vaccination data, School Contingency Plan, schedule of disinfection,
- School cleanliness and orderliness.
