In maintaining an updated personnel information, the field is hereby directed to accomplish and submit updated Civil Service Commission (CSC) Form 212 revised 2017 Personal Data Sheet (PDS) of all teaching and non-teaching personnel. PRC license holder personnel must also submit a photocopy of their valid PRC ID for validation. Enclosed is the guidelines for filling out of PDS.

All learning and development interventions/training programs attended until December 31, 2019 should be included in the PDS. The computerized CSC Form 212 must be subscribed and sworn by the following personnel:
a. School Principal/OIC/TIC — all teaching and non-teaching personnel in their respective schools; and
b. Mrs. Gladys G. Reyes, Administrative Officer V — all Division Office personnel and School Principal/OIC/TIC

The updated PDS must be submitted in two (2) printed copies with transmittal form and a soft copy saved in compact discs (CD) or Universal Service Bus (USB) flash drive per school on or before February 21, 2020 to the Personnel Unit. It is understood that the school copy is filed in the personnel’s
respective 201 file.
