Relative to the partnership between the Department of Education and Lions Club International, Inc, public school beneficiaries of the Sight for Kids (SFK) Project in Dasmarinas City are hereby informed of the schedule of implementation and other important details of the project.

Using the vision screening kits (Snellen chart, forms, etc) distributed by the Lions Club, class advisers, assisted by school clinic teachers, shall conduct the pre-screening of all Grades 3 to 6 pupils, along with selected Kindergarten to Grade 2 pupils with vision concerns and/or wearing prescription glasses. All
class advisers shall measure and record the visual acuity of students in SFK Form No. 1 (See Enclosure). The names of the students must be listed alphabetically.

All accomplished SFK Form No. I must be placed in a folder with the following information written on the front page: name of school, total population, total number of screened, total number of refracted, total number of awardees. The forms must be submitted in hard copy to the Health and Nutrition Unit do
Dr. Brenda Igos, Medical Officer III, on or before September 30, 2019 (Monday).

The submission of the required forms as well as the guidelines in conducting the visual acuity screening discussed during the orientation must be followed strictly. The next steps and schedule of refraction shall be announced accordingly.

Moreover, please be reminded that the conduct of all activities related to the SFK Project is subject to strict compliance with DepEd Order No. 9, s. 2005 on Instituting Measures to Increase Engaged Time-on-Task and Ensuring Compliance Therewith, and other existing DepEd rules and guidelines. Further, all
project costs shall be covered by Lions Club as a partner under the Adopt-a-School Program.
