This is to remind all School Heads and Adopt-a-School Program (ASP) Coordinators regarding the online submission of the School Partnership Data Sheet (SPDS) Third Quarterly Report, which covers all forms of ASP donations and support received from July 1 to September 31, 2018.
The SPDS template must be filled out and submitted in Macro-Enabled Worksheet format (.xlsm) via email to socmob.depeddasma.edu.ph on or before September 28, 2018.
In addition, a printed copy of the SPDS report signed by the School Head and the District Supervisor, together with the required attachments (i.e. Memorandum of Understanding/Agreement, Deed of Donation, and Deed of Acceptance), must be submitted to the SGOD office on or before the said date.
