1. In reference to DO 9, s. 2024 or the School Calendar and Activities for the SY 2024-2025, this office announces the administration of Multi-Factored Assessment Tool by Grade I teachers a month after the opening of classes as provided in DepEd Order No. 29, s. 2018, Policy on the Implementation of MFAT.

2. The policy aims to identify Learners with Special Educational Needs (LSENS), who may need educational services and/ or allied services; and to assist teachers in planning and designing instructions appropriate for LSENs.

3. As stated in the Policy Statement, this is a one-time assessment administered to Grade I learners who may exhibit developmental delays or with manifestation of learning disability.

4. This MFAT is a classroom activity-based assessment covering the five domains of learning:

a) Cognitive

b) Communication

c) Socio – Emotional

d) Psycho-Motor

e) Daily Living Skills

5. The deadline of submission of the said report is on or before September 30, 2024, through SDO- Dasmarinas City Records Unit using the attached template.
