1. The field is directed to accomplish and submit updated Civil Service Commission (CSC) Form 212 revised 2017 Personal Data Sheet (PDS) of all teaching and non-teaching personnel.
2. Professional Regulation Commission (PRC) license-holder personnel must also submit a photocopy of their valid PRC ID. The updated PDS form and the guidelines for filling out of PDS are downloadable at https://bit.ly/PDSformguide.
3. All learning and development interventions/training programs attended until February 28, 2025 must be included in the PDS.
4. Further, The CSC Form 212 must be subscribed and sworn by the following personnel as stipulated in Executive Order 292, Chapter 10 (Official Oaths), and RA 10755.
5. The accomplished PDS must be submitted in two (2) printed copy with transmittal form to the Personnel Unit on or before March 31, 2025. It is understood that one (1) copy is filed in the personnel’s respective 201 file.
