This is to remind all School Heads and Adopt-a-School Program (ASP) coordinators regarding the online submission of the ASP-SPDS fourth quarterly report, which covers all forms of ASP support (not to include the Brigada is Kwela donations) received from October 1 to December 20, 2017.
A printed copy of the SPDS signed by the principal and district supervisor, together with the required attachments (i.e. Memorandum of Understanding/Agreement, Deed of Donation, and Deed of Acceptance), must be submitted to the Division Office SGOD Unit and Legal Unit on or before December 22, 2017. All schools are expected to comply with this deadline.
