In conformity with the upcoming Fourth Periodical Examination scheduled on March 23-24, 2017, all elementary and secondary schools are advised to submit to this Office the copies of test questionnaire in all learning areas on or before March 15 duly signed by the school principal/Teacher In-charge (TIC) and Public Schools District Supervisors (PSDS).
