1. This is to inform the field that there will be an upcoming DENTAL MISSION for Grade 10 and Grade 11 of all Secondary Schools through the effort of our honorable Mayor Elpidio Barzaga Jr. through the City Health Office of Dasmariñas.
2. In this regard the CHO 1 and CHO 2 will conduct dental screening per High School. (Room to room set up)
3. Attached herewith are the forms to be used in the screening: -Dental Form -Consent Form
4. For students who need treatment, NO CONSENT, NO TREATMENT POLICY will be applied.
5. Please see the attached schedule under CHO 2. Schools under CHO 1, wait for further announcement.
