1. Relative to DepEd Memorandum No. 165, the Health and Nutrition Unit is requiring all schools to submit their Nutritional Status Report.
2. Please use the provided database of BMI Software given by the Central Office with attachment of photos of the height and weight taking for documentation. Deadline of submission is on January 23, 2019.
3. Submit the report through soft copy only to hnu@depeddasma.edu.ph
4. Immediate dissemination of and compliance with this Memorandum is directed.
