1. The City Schools Division of Dasmariñas through Health and Nutrition Unit will be conducting an Annual Physical Examination and Dental of Teaching and Non-Teaching Personnel.
2. In compliance to DepEd Order No. 44, S. 2004, the Health and Nutrition Unit shall vigorously pursue the implementation of the prevention and control of tuberculosis and other diseases through intensive case findings, health education, and treatment of all positive cases.
3. For the information of the field, 20-30 cases of personnel are diagnosed with cases of tuberculosis yearly. To help us find out who among us have this disease, all elementary and secondary teaching and non-teaching personnel are required to undergo chest x-ray and urinalysis.
4. In this regard, all teaching and non-teaching personnel must provide the chest x-ray and urinalysis result during their APE scheduled date. The Health and Nutrition Unit will not provide/ endorse any laboratory clinic.
5. Attached herewith is the schedule along with the format of the Annual Physical Examination (APE) Masterlist. APE Masterlist must be submitted in hard and soft copy- thru our official email, hnu@depeddasma.edu.ph
