1. In reference to Memorandum (DM-PHROD-2021-0165) dated March 04, 2021, this Office shall gather feedback forms from April 12-17,2023. This is in line with the goal of the new leadership to provide clients and stakeholders with excellent service delivery.

3. The feedback gathered shall be consolidated and used as one of the bases of this Office to improve systems, approaches, and services in the City Schools Division of Dasmarinas.
