1. Pursuant to the previously released Memorandum from the Office of the Undersecretary for Human Resource and Organizational Development and Infrastructure dated January 23, 2026 and Regional Memorandum No. 69, s. 2026 pertaining to the above-mentioned subject, this Office hereby informs the field of its implementation.
2. The Field Technical Assistance Division (FTAD) and School Governance and Operations Division (SGOD) was given the task to ensure the widest dissemination of this Memorandum. The Google Survey Form may be accessed through tinyurl.com/SBMFY26-SurveyForm.
3. The School Head shall accomplish and submit the said Survey. In cases where a School Head supervises more than one (1) public school, only one survey form must be submitted. All respondents shall ensure accurateness and completeness of all basic school information. They are advised to review their responses carefully prior to final submission using official DepEd email addresses.
