1. In compliance with DepEd Order No. 44, S. 2004, the Health and Nutrition Unit shall pursue vigorously the implementation of the Prevention and Control of Tuberculosis, and other diseases through intensive case findings, health education and treatment of all positive cases. To help us find out who among us have these diseases, all elementary and secondary teaching and non-teaching personnel are required to undergo Chest X-ray and Urinalysis.
2. The City Schools Division of Dasmariñas through Health and Nutrition Unit will be conducting the Annual Physical Examination of Teaching and Non-Teaching Personnel from July 2022 to August 2022 with a schedule to follow.
3. All Teaching and Non-Teaching Personnel in the division must provide the Chest X-ray and Urinalysis result during their APE scheduled date. The Health and Nutrition Unit will not provide/ endorse any laboratory clinic.
4. Attached herewith is the format of the Annual Physical Examination (APE) Masterlist. APE Masterlist must be submitted in hard and soft copy thru our official email at hnu@depeddasma.edu.ph
