1. Pursuant to DepEd Memorandum No. OM-OAGA-2019-00176 otherwise known as “Administration of Survey Form for Selected Schools Heads for the Possible Inclusion of Alternative Learning System in the School Based Management System (SBM)” the selected schools with ALS program are hereby required to fill up the survey form attached in this memorandum and submit to the Division ALS Focal Person on or before 12:00 noon of July 31, 2019.
