1. Relevant to the letter received from the City Local Government Operations re: Meeting on Child Friendly Local Governance Audit (CFLGA), the field is requested to submit data on Out-of-School Children and -Youth Reintegrated in Formal School for School Year 2018-2019.
2. All public schools shall complete the report template available at www.deped.in/yfddasma or www.tiny.cc/yfddasma with the following information:
a. Total number of school dropouts reintegrated in formal school by Grade Level
b. Total number of school dropouts aged from 5 to 17 years old
3. The deadline of submission is on September 6, 2019.
