1. Relative to Division Memorandum No. 213, s. 2024 titled Official Examinees for Fiscal Year 2023 National Qualifying Examination for School Heads (NQESH), and to fill in the remaining six (6) NQESH examinee slots allocated for the CSDO, this Office would like to request all interested and qualified applicants who meet the experience requirement HEAD TEACHER FOR AT LEAST ONE (1) YEAR to submit the following documentary requirements to the CSDO-NQESH Team through the Records Unit in 1 original copy and 1 photocopy fastened in a long folder with attached Records Request Form not later than March 15, 2024, 1:00pm:
a. Approved Results-based Performance Management System (RPMS) Individual Performance Commitment and Review Form (IPCRF) with a rating of at least Very Satisfactory (VS) in the last two (2) consecutive rating periods (SY 2021-2022 and SY 2022-2023) duly certified by the authorized personnel in the SDO; and
b. Service Record duly certified by the Administrative Officer V of the Schools Division Office.
2. Since this is only a reopening of the submission of documentary requirements, only the first six (6) qualified submissions shall be accepted by the Records Unit and validated by the CSDO-NQESH Team.
