1. In reference to DepEd Memorandum No. 542 S. 2024, entitled “Dissemination of the Office Memorandum Order No. 397-2-2024 (Pambansang Pabahay Para sa Pilipino (4PH) Program)”, the Urban Poor Affair Office (UPAO) through the City Schools Division of Dasmarinas invites the select teaching and non-teaching personnel (please see enclosure 1) to attend the conduct of 4PH Orientation on the assigned dates per cluster.
2. The orientation shall focus on the dissemination of the Pambansang Pabahay Para sa Pilipino (4PH) Program framework and guidelines, the briefing of pre-qualified teaching and non-teaching personnel on program benefits, requirements, and processes, and the clarification of roles, responsibilities, and subsequent actions to be undertaken by the participants. Moreover, the select personnel are requested to bring the following:
a. Two (2) Valid ID with specimen signature
b. Birth Certificate, if Single
c. Marriage Certificate, if Married
d. Payslip in the last three (3) months
e. Certificate of Employment, if employed
f. Special Power of Attorney, if applicable
g. Authorization to conduct Credit Investigation
3. Attendance in this activity is voluntary. However, all prequalified personnel are highly encouraged to participate to be properly guided on the updating process and to ensure the accuracy of submitted information.
