1. Relative to a letter received from Mr. Jose Luis F. Decena, Social Welfare Assistant of City Operations Office (C00) – Dasmaririas, this Division informs all public elementary and secondary schools that the Field staff of 4Ps in Dasmariñas City will distribute the Compliance Verification Forms (CVFs) used in monitoring the educational condition of the program in a soft copy through a google link to be sent on the official 4Ps Coordinators group chat.
2. All 4Ps school Focal persons are expected to facilitate the accomplishment of the said CVFs and shall submit the same to the SGOD Office on or before April 3, 2025 (12NN).
