1. In consonance with the grant of Educational Assistance for Senior High School students under the Department of Social Welfare and Development which will be implemented by the City of Dasmariñas, the field is hereby provided with the herein list of requirements to be accomplished and submitted by Grade 11 and 12 senior high school students who are 18 years old and above.
2. If the student is below 18 years old, his/her mother/father should accomplish the General Intake Sheet and the Certificate of Eligibility. Guardians may accomplish the form only when the father/ mother is not living in the same residence as the child and provided that the guardian is either the parent or sibling of any of the parents of the child.
3. The requirements shall be submitted to Ms. Almira Torre, Social Welfare Officer II of DSWD IV-A at the Covered Area between APD & DASCA Building following the indicated time and schedule of senior high schools listed in the attachment.
4. To ensure the smooth submission of document, this Office hereby enjoins all senior high school principals to disseminate the information to the concerned parents and students for guidance and appropriate action.
5. Immediate dissemination and compliance with this Memorandum is directed.
